Reference Forms
Members serving K-8 th grade students in Philadelphia, PA, are required to provide two character references and an employment verification of experience with children and youth. This is a licensing requirement.
Character References
- 1. Two (2) character references are required. Relatives may not provide references.
2. The Reference Form may be used and mailed directly to EducationWorks.
3. A reference letter written within 6 months of starting with EducationWorks may also be used.
4. The form or letter must include a non-electronic signature. - Employment Verification
1. One employment verification is required. The Employment Verification Form must be used.
2. The position described on the form should reflect:
-- Experience with children or youth. This may include, but not limited to experience in an afterschool
program, youth group, child care facility, extensive babysitting.
-- Paid or unpaid experience.
-- The most comprehensive or longest experience; it does not need to be the most recent experience.
-- If you are unable to obtain a verification related to working with children or youth, please provide a verification from your most recent employer.
3. The form must be completed in full and include a non-electronic signature.
